USA Hockey membership

NorCal Extreme players are covered under USA Hockey Inline insurance. USA Hockey insurance is now used by Davis Inline Hockey Association (DIHA), Davis Blue Devils Hockey Club (Davis High School Hockey), and Extreme. USAHI insurance is also required at many tournaments. Thus, the annual USA Hockey Inline membership ($30, expires 31 August annually) will cover most of your hockey adventures. Note that USA Hockey Ice membership is separate and unfortunately does not cover inline hockey.

If a player has participated in the fall DIHA session or this autumn's Davis High School Blue Devils Hockey Club (JV or varsity), then s/he probably already has a USA Hockey Inline membership. Simply include your membership number on the Extreme Registration Form.

If you need to apply for USA Hockey Inline membership, you may register online (and tell your team representative about your USAHI membership number),

2. Fill the second page. Do not forget to sign the form.
3. Give the form and a check for $30 made out to "NorCal Extreme"
   to your team representative (do not mail it to Colorado!)
